Cumberland Heights Admissions Assessment

Before your initial visit to Cumberland Heights, you will receive a comprehensive admissions assessment with a licensed intake counselor which helps us determine the appropriate course for your treatment. These are usually conducted over the phone and take about 10-15 minutes. Accompanying family members or friends can and should meet independently with an intake staff member for a brief interview and to have any questions answered regarding treatment as well.

    Authorization Form for Release of Patient Information

    All requests for Health Information from Cumberland Heights must be either be A) Notarized or B) Presented with a copy of Photo ID of the patient. The PATIENT must initial next to EACH ITEM and sign and date the request. This is our standard authorization for release of patient information. The purpose of this disclosure of information is to improve assessment and treatment planning. The sharing of information is relevant to treatment and when appropriate, to coordinate treatment services and continuing care planning. The completed form can be faxed to Cumberland Heights Medical Records at 615-432-3291, or scanned and emailed to MedicalRecords@cumberlandheights.org.

  2. Insurance & Financial Matters: Financial arrangements are typically agreed upon prior to your scheduled admission. Our insurance relations specialists will assist you in maximizing the benefits available under your health insurance plan.   3. What to Bring / Not to Bring: Before you pack, review our list of suggested items to bring for your stay at Cumberland Heights. We ask you to bring all prescriptions and any over-the-counter medications in their complete packaging including prescription bags. The medical staff will safely store them until one of our physicians is able to determine their use during and after treatment.
